Referral Information for Medical Practitioners
This section is for General Practitioners (GP’s) and Medical Specialists which hold a Medicare Medical Provider number and are eligible to refer Patients to a Medical Specialists
The electronic referral can be completed online and submitted or printed and faxed, we will then contact the patient and make an appointment.
Ear and Hearing conditions including dizziness: We require patients with ear or hearing conditions to have a recent hearing test including bone and air conduction before their appointment, we are happy to arrange this. Please indicate on the space available on the referral.
